Document Check List
- Government-issued photo ID, proof of address
- Monthly rent and amount past-due
- Copy of lease or rental agreement
- Proof of household income in 2020
- Tax returns W-2s, 1099s, paystubs
- Income attestation (if paid in cash)
- Proof of public assistance (if applicable)
- Proof of immigration status is NOT required
* Household has experienced a financial hardship due to the pandemic / Under 80% of the Area Median Income
* SSN or ITIN is not required for tenants to apply – If lease is not available, the landlord can write an affidavit
* Examples of hardship due to COVID-19; Out of work, reduction in hours at work, loss of spousal/child support.

Not be discriminated against:
- Landlords cannot use your immigration status to intimidate or threaten to call ICE, evict you or shut off utilities.
A habitable home
- You have a right to essential services and emergency repairs for heat, water, and electricity.
Privacy
- Except for emergencies, landlords cannot enter your home without notice: Cook County (48 hours notice) Outside of Cook County (24 hours notice)
Receive proper notice
- Landlords must provide 30, 60 or 120 days’ notice to not renew a lease, even if it is month-to-month. Document all communication with your landlord in writing!
